The Lenawee Christian Athletic Department partnerships with FinalForms, an online platform for athletic forms and data management.
Key Benefits:
- Complete and sign all athletic participation forms online.
- Save time: Information carries over from season to season and year to year—no need to re-enter data.
- Smart autofill: FinalForms pre-populates information for each of your students.
- Access physical forms easily: Download and print the MHSAA physical examination form directly from your FinalForms account to take to your athlete’s doctor—no need to pick one up from school.
- Improved communication: Coaches and the athletic office will use FinalForms to send program-specific updates to students and parents.
- Summer workouts: Students must be registered in FinalForms before participating.
- Athletic Commitment Form included: This form is now part of the registration process and no longer sent separately.
